Insurance & Billing
Insurance
We accept most major PPO plans, including Blue Cross Blue Shield, UnitedHealthcare, Cigna, Aetna, UMR, and Meritain Health. Coverage varies by plan. Please call us and we'll help verify benefits.
Not Accepted
We do not accept HMO plans. These plans are generally restricted to a pre-specified coverage network and will not cover any services rendered by us.
Self-Pay Services
Self-pay services are not billable to insurance plans and include:
Medical weight loss programs
Immigration medical exams (I-693 and N-648)
Low testosterone (TRT)

Financial Policy Highlights
Copays are due at check-in; deductibles/coinsurance and non‑covered services are due at the visit or upon statement.
If your insurer hasn’t paid within 60 days, any remaining balance becomes your responsibility.
Professional services are non‑refundable; overpayments are refunded after insurance finalizes the claim.
